Instructions

1. Show engines in numerical order. Use prefix "x" to indicate engines operating under extension for retubing.

2. Show sheet number at the top of each page.

3. Service to be indicated thus:

	P-Passenger		F-Freight	S-Switch
	W-Work			LU-Laid Up	UR-Under Repairs
	WR-Waiting Repairs	LSD-Leased	REM-Removed From Service

4. Condition to be determined by:

	Good:
	Mileage basis and tires

	Moderate:
	Mileage basis and tires

	Poor:
	1.Mileage basis and tires
	2.Under and waiting No. 5 or heavier repairs
	3.Outlawed next 3 months if extension is not going to be applied for

5. Condition and mileage to be inserted in office of Supt. M.P.& C.E.

6. Condition according to mileage since last no.5 or heavier to be determined by classes as follows:

   Measurements to be made to the nearest one-sixteenth 1/16" of an inch.

8. Condition of each individual engine to be indicated by an "X" in the proper column.

9. Thickness of tires shall be indicated as at last turning, until again turned.

10. Remarks column shall be used to:

	A. Show present location of engine if at other than the station to which assigned
	B. Thickness and location of badly worn flanges
	C. Show major defects not otherwise called for by form and other items of interest
	D. Show class of repair for engines under and awaiting repairs and if an engine is outlawed, to
	   indicate same by letters "OL"

11. Engines stored servicable to be listed for condition same as engines in service and shown seperately at end
    of report.

12. Engines stored unservicable to be classed as poor and listed seperately at end of report.

13. Engines will be considered as removed from service when held waiting to be sold or scrapped. Condition not
    to be shown but engines to be listed seperately and shown at end of report.

14. Total number of locomotives assigned to district to be shown in the space provided on the last sheet onlY
    of monthly report.

15. Engines receiving a No. 5 repair or better at other then main shops. A notation must be made in remarks
    column showing class of repair and station.
